product failureRAH Consultancy will encompass investigations into product failure as an independent expert who has knowledge of, and access to independent Fitting and Materials Testing Laboratories capable of undertaking testing to expert’s requirements to assess reasons for failure. This work has been a mainstay of his work in past employment and can be assessed normally against the quality requirements of the British and European Standards appropriate to the device unless particular requirements were required by the supply contract. The work invariably entails an inspection of the premises and method of installation as much product failure can be wholly or partly to do with incorrect installation.

Product failure can also be of a contractual nature. Contracts have arisen where the item supplied was not as specified, for instance a gate valve was required by the purchaser to meet the requirements of BS 5163. The supplier awarded the contract was suspected of providing valves not as required under the contact and inspection and testing against the specification proved this was so.

 
This work often forms part of the preparatory work for expert witness reports and some of the cases for failure of pipes or systems are included under these cases. RAH has been involved in excess of 500 of these cases.
